1. Who we are and who we are not

Fortis Disability is a disability benefits advocacy company. We help people apply for and appeal SSDI and SSI claims. We are a private company. We are not part of the Social Security Administration (SSA), and SSA does not endorse us.

2. What this site does

This site gives general information and a quick check of whether we are able to help. It is not a decision about your claim. SSA makes all decisions about benefits. Information on this site is general and is not advice about your situation.

3. Agreeing to these Terms

You agree to these Terms when you check the box on one of our forms or when you hire us. If you only browse, these Terms still apply to your use of the site.

4. No guarantees

We do not guarantee approval, a benefit amount, or a decision date. Past results do not predict future results. Stories on our site come from real clients, and results may vary.

5. Becoming a client

We are not your representative until you sign a written representation agreement and complete SSA's appointment of representative form (Form SSA-1696). The agreement describes our services. If these Terms conflict with your agreement about our services, the agreement controls.

6. Your responsibilities

• Give us accurate and complete information.
• Tell us about changes to your work, address, medical care, and hospital stays.
• Send us SSA letters right away, and answer SSA requests on time.
• Do not give false information to us or to SSA. False statements carry penalties.

7. Calls and texts

Section 6 of our Privacy Policy explains calls and texts. Reply STOP to end texts.

8. Our content

The site and its content belong to us or our licensors. You are free to view and print it for your own use. Do not copy it for other uses.

9. Links to other sites

We link to other sites, such as ssa.gov. We do not control them and are not responsible for them.

10. Limits on our liability

We offer the website and its general content "as is." These limits apply only to your use of the website. They do not limit our responsibility for services we provide as your representative, which your representation agreement and the law govern.

11. Disputes

If you have a concern, contact us first at contact@fortisdisability.com or 866-448-5216. We will try to resolve it quickly and respectfully. If we do not, either side is free to bring a claim in the courts of Contra Costa County, California or in small claims court. These Terms contain no arbitration clause and no class action waiver. These Terms are governed by the laws of California.

12. Changes

We post updated Terms here with a new date. For material changes, we email you or show a notice at least 30 days before they start. Changes do not apply to disputes already started.

13. Accessibility

If you have trouble using this site, email contact@fortisdisability.com or call 866-448-5216 and we will help you.

14. General

If a court finds part of these Terms unenforceable, the rest stays in effect. You are not able to assign these Terms without our written consent.
